Whippet Application & Questionnaire

If not planning to show your dog, are you willing to spay/neuter him/her?
Please check any activities, if any, in which you plan or would like to participate with your Whippet :
Should your circumstances change so that you were no longer able to care for this dog, will you agree to return the dog to us?

In the space provided below, please answer the following questions:

1.   Number of people in the household and their ages:

2.  Are all members in the household in agreement with adding a Whippet to the family?

3.  Who will be the primary caretaker of the dog? What is their occupation (in general).

4.  Do you allow pets on your furniture?

5.  What type of home and community do you live in (House/Apartment, Rent/Own)? How long have you lived here?

6.  What type of fencing and how tall is it? (block, chain link, invisible, etc.) If you don't have a fenced yard how will you exercise your whippet?

7.  Please tell us about any other pets you have. If dogs, how many, breeds or mixes, ages, whether inside or outside dogs, and whether they are neutered or spayed. If cats, whether they are indoor or outside, and ages?

8.  How many dogs have you owned in the past 10 years? Would you tell us what happened to them?

9.   Is it alright if we call your veterinarian for a reference? If so, please provide his/her name and phone number: 

10.  How did you decide on a whippet as the breed for you?

11.  Will someone be home with your Whippet during the day? If not, how many hours will he be home alone?

12.  Where will your dog stay while you're away during the day? Where will your Whippet sleep at night?
